Spinal Manipulation Therapy (SMT) is a hands-on treatment technique commonly used in Ayurveda, Chiropractic, Osteopathy, and Physiotherapy. It involves applying controlled force to specific joints of the spine with the goal of restoring mobility, relieving pain, and improving overall body function. In Ayurveda, it’s often combined with oil massage (Abhyanga), heat therapies, and Panchakarma for better results
Step-by-Step Method (General Traditional Approach)
1. Initial Consultation & Assessment
Practitioner checks patient’s medical history, posture, flexibility, and spinal alignment.
Identifies areas of stiffness, pain, or restricted movement.
2. Preparation
Patient lies in a comfortable position (face down, sideways, or seated).
Warm herbal oils may be applied (in Ayurvedic method) to relax muscles and lubricate joints.
Sometimes steam therapy (Swedana) is done before manipulation to soften tissues.
3. Positioning
Patient is placed in a specific posture depending on the area of the spine (neck, thoracic, lumbar, or sacral region).
Practitioner ensures correct alignment before applying manipulation.
4. Manual Adjustment / Manipulation
Controlled, sudden but gentle thrust is applied with the hands (sometimes elbows).
A popping/cracking sound may occur (release of gas bubbles from joints).
Movements may include:
Rotation – twisting motion for spinal release.
Flexion/Extension – bending forward/backward.
Lateral bending – side adjustments.
Distraction technique – gentle pulling to stretch vertebrae.
5. Post-Manipulation Relaxation
Patient rests for a few minutes.
Light massage or herbal application may be given to relax tissues.
6. Aftercare Instructions
Drink warm water / herbal tea to aid detox.
Avoid heavy lifting or sudden movements for 24 hours.
Gentle stretching and breathing exercises may be advised.
